MILWAUKEE – Right-handed pitcher Shelby Miller is facing the possibility of undergoing his second Tommy John surgery, according to an announcement from the Milwaukee Brewers. The Brewers have placed the veteran on the 60-day injured list due to a sprain of the ulnar collateral ligament in his throwing elbow. Miller spoke with the media on Wednesday, revealing that he plans to consult with Dr. Keith Meister to discuss the results of his MRI and define the next steps. The pitcher acknowledged that surgery seems to be a realistic option and reflected on his future in baseball.

I will definitely, probably miss next year. I’ll be ready for 2027, I’ll see how my arm feels, where I am in my life, and I’ll make decisions from there.

Miller, who will turn 35 next month, already underwent Tommy John surgery in 2017. Milwaukee manager Pat Murphy reported after Monday’s 10-8 loss to Philadelphia that Miller “felt something in his elbow” while pitching in the eighth inning. Miller was immediately removed from the game.

This is part of the game. Baseball demands a lot of physical wear and tear. You play for a long time, like me, and there will be injuries and setbacks. It’s difficult.

In a corresponding move, the Brewers, leaders of the National League Central Division, called up right-handed pitcher Joel Payamps from Triple-A Nashville. Miller joins All-Star closer Trevor Megill (right flexor strain), left-handed pitcher DL Hall (right oblique) and right-handed pitcher Grant Anderson (right ankle) on the Brewers’ injured list.

Murphy expressed his support for Miller.

I feel so bad for him. He’s a veteran who comes to a new team, earns everyone’s respect immediately, is assigned a high-pressure position, and does well almost always. With the number of injuries in our bullpen, it affects us a lot. But I feel worse for the player.

Milwaukee, with the best record in Major League Baseball, leads its division by five games over the Chicago Cubs. Miller intended to play an important role in the Brewers’ fight for the playoffs, seeking their first World Series title in franchise history. Instead, he will have to consider his long-term plans. He was asked on Wednesday if retirement was a possibility. Miller said he is confident he can still get batters out and that he might have a chance after surgery, but he also acknowledged the difficulty of a comeback.

A second TJ is never easy. It will depend on how I feel and where I am in my life when that day comes. We’ll see.

With the Diamondbacks, he had a 0-3 record with an ERA of 1.98 in 37 games. Payamps, 31 years old, played a key role in Milwaukee’s bullpen from 2023 to 2024. This season, he has a 0-1 record with an 8.35 ERA in 23 appearances with the Brewers.
