Irregularities revealed in the Cobra 2.0 case: Medical authorizations under suspicion at Senasa

The judicial case known as Cobra 2.0 has brought to light fraudulent maneuvers involving the Dominican health system. As detailed in the Public Ministry’s file, irregular maneuvers were identified, executed from the Medical Authorization Department of the National Health Insurance (Senasa), where physicians’ credentials were used to approve procedures without the patients’ consent.

One of the most revealing episodes occurred on December 3, 2024, when Ángel Luis Guzmán, an assistant in said department, attempted to manage the authorization for a radiofrequency cervical cauterization. The procedure, which is gynecological in nature, was requested using the code of Dr. Carlos Job Ynoa Olivero. The alert was triggered when the call center representative requested the patient’s information, at which point Guzmán chose to end the call abruptly without completing the request.

Medical inconsistencies and false billings

The file qualifies this fact as an objective medical inconsistency, underlining that there was an attempt to validate interventions with data that did not relate to the affiliate’s clinical profile. The Public Prosecutor’s Office maintains that this practice was part of a larger scheme to defraud the state insurer.

Among the most serious findings presented by the authorities are:

  • Gynecological consultations systematically registered under the names of male affiliates.
  • Billing for medical services to patients who denied having received care or knowing the professionals involved.
  • Use of falsified clinical records and claims to obtain irregular payments.

The case of citizen Odalis Suárez de los Santos exemplifies the network’s modus operandi. Medical services from practitioners she has never visited or met were charged to the affiliate, including Dr. Carlos Job Ynoa Olivero and a group of doctors identified as part of the criminal structure.

In addition to administrative irregularities, the authorities have proceeded with the seizure of various assets linked to the network, including high-end vehicles, luxury watches, and sums of cash. Investigations are ongoing to determine the full extent of this fraud, which affects the integrity of the public health system in the country.
